  • DO I NEED A PRESCRIPTION FROM A DOCTOR?
    In the state of Illinois, physical therapy services can be provided without a physician referral. If the patient is under the care of a physician for the issue, they must be notified within 5 days of the PT evaluation. If the patient is not making improvements in 10 visits or 15 days, the patient must be referred to a physician of their choice.
  • HOW DOES PAYMENT WORK?
    Payment for physical therapy services is due in full at the time of service. The credit card on file will be charged at the time of service. Integrated Movement Physical Therapy has chosen to work outside of commercial insurance plans. Not being a preferred provider with insurance companies allows Integrated Movement PT to provide you with the highest quality of care. Dr. Jill Maida will create an individualized treatment plan that is tailored to your specific needs, not limited by insurance company restrictions.
  • CAN I SUBMIT OUT-OF-NETWORK CLAIMS TO MY INSURANCE?
    Documentation will be provided with the necessary information for your to bill your "out-of-network" benefits if you choose to. Integrated Movement Physical Therapy makes no guarantee that charges incurred will be reimbursed.
  • WHAT IF I HAVE MEDICARE?
    Integrated Movement Physical Therapy is not enrolled with Medicare. This means that we are not permitted to perform any covered services for patients who have Medicare. We can provide services that address fitness, wellness, prevention, or maintenance as these services are not covered by Medicare. This means that Medicare will not reimburse for physical therapy services at Integrated Movement Physical Therapy. Claims cannot be submitted to Medicare or a secondary insurance provider.   • HOW MANY SESSIONS WILL I NEED?
    The number of physical therapy sessions will be decided on by you and the therapist at the evaluation. Most issues can be addressed with one visit per week. We can increase or decrease your number of sessions based on your progression. Since no referral is needed to access physical therapy in Illinois, you can return to therapy anytime you feel the need to do so.
  • WHAT SHOULD I WEAR?
    The most important thing is that you are comfortable and can move easily. Bring tennis shoes and any orthotics you use. If there is inclement weather, please wear alternative shoes and change into your tennis shoes before entering the clinic. There is a small waiting area/bathroom for your convenience.
  • DO YOU HAVE PARKING?
    Park on the street in front of the Lombard location. There is one step to enter the premise. If you feel you need assistance to enter, please reach out on the PT Everywhere App and I will be happy to assist.
  • WHAT IS YOUR CANCELLATION POLICY?
    The credit card on file will be charged a $75 fee if you do not cancel your appointment 24 hours before of your appointment time.
